Trista Bailey

Dr. Trista Askins Bailey is an Associate Professor in the Geriatric Division – Department of Pharmacy Practice at the Jerry H. Hodge School of Pharmacy at Texas Tech University Health Sciences Center in Abilene, TX. She is involved with didactic lessons, laboratory teaching, facilitating case-based learning, and Geriatric clerkships. Dr. Bailey currently has a practice site as a consultant pharmacist for two nursing homes and an assisted living facility in the Abilene area, where she looks after around 120 patients. Her duties include monthly chart reviews, anti-infective and psychotropic drug monitoring, medication pass observations, nursing education, and drug destruction. Dr. Bailey received her Bachelors of Science with Honors from Texas Tech University in 2007 and her Doctorate of Pharmacy from The University of Texas at Austin/The University of Texas Health Science Center at San Antonio dual degree program in 2011. She then went on to complete an ASHP-accredited PGY-1 ambulatory care/geriatrics focused residency with the Battle Creek VA Medical Center. She is dual board certified in both Pharmacotherapy (BCPS) and Geriatrics (BCGP). Dr. Bailey is also a fellow of the American Society of Consultant Pharmacists (FASCP).

Amie Blaszczyk

Amie Taggart Blaszczyk, Pharm.D., BCGP, BCPS, FASCP is a Professor of Geriatric Pharmacy Practice and the Division Head of Geriatrics at the Texas Tech University Health Sciences Center School of Pharmacy on the Dallas/Fort Worth campus. She has an affiliated practice with Advanced Healthcare Solutions, a long-term care company with ownership of over 40 nursing facilities across the state of Texas. There she focuses on the population level interventions, which can positively affect the quality of care of long-term care residents. She is also the residency director for the ASHP-accredited PGY2 Geriatric Pharmacotherapy residency.

Dr. Blaszczyk earned her Doctor of Pharmacy from Duquesne University and completed a pharmacy practice residency at the VA in Milwaukee, Wisconsin and a PGY2 geriatric pharmacotherapy residency at the University of Maryland in Baltimore. Her practice and research interests include neurodegenerative diseases affecting older individuals, senior education and the impact of a clinical pharmacist working in the long-term care setting. She is the author of 22 peer-reviewed publications, 2 book chapters, and has been asked to speak on various geriatrics-focused topics at both international and national meetings.

She is the past-president of the Texas chapter of the American Society of Consultant Pharmacists, serves on the editorial board of The Consultant Pharmacist and is the 2018 recipient of the Armon Neel Senior Care Pharmacist award. She is also an active member of the American Society of Health-Systems Pharmacists, currently serving on the Council on Therapeutics and is a senior-level contributor to the ASHP/ACCP BCGP course and materials for recertification credit.

Dana CarrollDana G. Carroll, Pharm.D., BCGP, BCPS, CDE, is a Clinical Professor at Auburn University Harrison School of Pharmacy, Auburn, AL. She graduated from Auburn University School of Pharmacy with her BS (1998) and Pharm.D. (1999). She completed a residency in primary care at The University of Mississippi Medical Center in 2000. She worked at Duke Medical Center in the emergency room as a clinical pharmacist and as an Assistant Professor at University of Oklahoma (Tulsa campus) before joining the faculty with Auburn in 2006 as an Associate Clinical Professor. Her current clinical practice at University Medical Center in Tuscaloosa, AL focuses on providing direct patient care as part of interdisciplinary teams in a geriatric clinic and a transition of care clinic (which primarily serves geriatric patients). In addition, Dr. Carroll provides one on one patient education and drug therapy consults in diabetes and serves as part of an interdisciplinary team to deliver ADA certified diabetes self-management classes. She is board certified in pharmacotherapy (BCPS) and geriatrics (BCGP). She also is a certified diabetes educator (CDE). She teaches and precepts pharmacy students, pharmacy residents, graduate students (psychology and social work) and medical residents. She also holds adjunct faculty appointments with the College of Community Health Sciences Family, Internal and Rural Medicine Department and the Psychology Department at the University of Alabama. Dr. Carroll’s current scholarly endeavors include assessing outcomes in geriatric patients from interdisciplinary care and the scholarship of teaching and learning in both didactic and clinical settings.

Collin Clark, PharmD, BCPS, BCGP

Collin ClarkCollin M. Clark is a Clinical Assistant Professor in the Department of Pharmacy Practice at the University at Buffalo School of Pharmacy and Pharmaceutical Sciences (UB SPPS). He completed his Doctor of Pharmacy at the UB SPPS in 2017. He then went on to complete a PGY-1 Pharmacy Practice Residency at the Veteran Affairs Western New York Healthcare System and a T32 Health Services Research Fellowship at the University at Buffalo Jacobs School of Medicine and Biomedical Sciences. He currently practices as a Clinical Pharmacist in Transitions of Care at Buffalo General Medical Center. His research centers on the implementation and assessment of pharmacist provided clinical services. He has a particular interest in studying the pharmacist’s role in transitional care management across the continuum of care and medication optimization strategies for older adults with complex medical conditions.

Kalin CliffordKalin Clifford, Pharm.D., BCGP, BCPS, is an Associate Professor of Pharmacy Practice, Division of Geriatrics at Texas Tech University Health Sciences Center School of Pharmacy in Dallas, Texas. His current practice site is within the North Texas Veterans Affairs Hospital within the Community Living Center (CLC-A), a sub-acute rehab facility. Dr. Clifford also precepts students on the required Geriatric Clerkship rotation during their Advanced Pharmacy Practice Experiences (APPE) year. Prior to beginning his career at Texas Tech, Dr. Clifford received his degree from Butler University College of Pharmacy and Health Sciences in Indianapolis, Indiana in 2011. He also completed a PGY-1 in Pharmacy Practice from Columbus Regional Hospital in Columbus, Indiana in 2012 and a PGY-2 in Geriatric Pharmacy Practice from the University of Maryland, School of Pharmacy in Baltimore, Maryland in 2013. Emily EllsworthEmily Ellsworth, Pharm.D., BCGP, received her doctorate degree from Butler University in Indianapolis, IN. She further completed her PGY1 Pharmacy Practice Residency at the Edward Hines, Jr. VA Hospital in Chicago, IL. Her passion for the aging population led her to the University of Arkansas for Medical Sciences (UAMS) College of Pharmacy where she completed her PGY2 Geriatric Pharmacy Residency. Her professional interests include geriatric ambulatory care, long-term care, and academia. She is a Board Certified Geriatric Pharmacist (BCGP) who currently works as a Clinical Pharmacy Specialist at the Edward Hines, Jr. VA Hospital. Her current role in Home-Based Primary Care (HBPC) allows her to work directly with geriatric veterans providing disease state management in their homes. She is also heavily involved with the PGY1 Pharmacy Practice Residency program and precepts students from Midwestern University.

Carol Fox

Dr. Carol Fox is an Associate Professor and Chair at the University Of South Florida Taneja College Of Pharmacy in the department of Pharmacotherapeutics and Clinical Research. She is a consultant pharmacist at an independent living center and a local skilled nursing facility where she serves as a preceptor for advanced practice pharmacy students on their Geriatric rotation. She is course coordinator for the required Geriatric Pharmacotherapy course and also teaches geriatric related topics in the Pharmacotherapeutics course sequence. Dr. Fox obtained her Pharm.D. from the University of Connecticut School of Pharmacy and completed a Geriatric Pharmacy Practice residency at the Durham VA Medical Center in North Carolina. Dr. Fox is a Board Certified Geriatric Pharmacist.

Emily Frederick

Dr. Emily Frederick received a Bachelor of Science degree in Neuroscience from the College of William and Mary in Williamsburg, Virginia. She earned her Doctor of Pharmacy degree from Shenandoah University Bernard J. Dunn School of Pharmacy in Winchester, Virginia and completed a pharmacy practice residency at Geisinger Medical Center in central Pennsylvania. Dr. Frederick is an associate professor and the Vice Chair of the Pharmacy Practice Department at Sullivan University College of Pharmacy and Health Sciences in Louisville, Kentucky. She practices as a clinical pharmacist at the University of Louisville Hospital as part of the Emergency Department Hospitalist Team. Dr. Frederick is active in local and national professional organizations and serves on the Board of Directors for the Kentucky Society of Health-System Pharmacists. In her free time, Dr. Frederick enjoys spending time with her husband and two children, baking, and evaluating literature.

Lisa Hutchinson

Lisa Hutchison is Vice Chair and Professor in the Department of Pharmacy Practice at the University of Arkansas for Medical Sciences with a secondary appointment as Professor in the UAMS College of Medicine. Dr. Hutchison earned her BS Pharmacy and PharmD degrees at the University of Tennessee Center for Health Sciences and a Master of Public Health degree from the University of South Florida. She is dual Board Certified in Pharmacotherapy and Geriatric Pharmacy.

Dr. Hutchison is a Fellow of the American College of Clinical Pharmacy. She won the Award for Achievement in the Professional Practice of Pharmacy in Organized Health Care Settings from the American Society of Health system Pharmacy (1993) and the American Pharmacists Association’s Distinguished Achievement Award in Hospital and Institutional Practice (2008). She is the editor of Fundamentals of Geriatric Pharmacotherapy, 2nd edition and an annual lecturer for ASHP/ACCP’s Geriatric Pharmacy Certification Course.

Jill Johnson

Jill R. Johnson, Pharm.D., M.H.A., BCPS, is Clinical Pharmacy Specialist at the Veterans Affairs Medical Center in Memphis, Tennessee. As a clinical pharmacy specialist with the Veterans Affairs Medical Centers in Lexington, Kentucky, and Memphis, Tennessee, her roles have included providing geriatric clinical pharmacy services to veterans in the Community Living Center and Home Based Primary Care programs, providing pain and symptom management services to hospice and palliative care patients as a member of the Palliative Care Consult Team, and providing ambulatory care clinical pharmacy services to veterans in the Community Based Outpatient Clinic.

Dr. Johnson earned her Doctor of Pharmacy degree from the University Of Tennessee College Of Pharmacy, completed a Pharmacy Practice Residency with emphasis in geriatrics at the Veterans Affairs Medical Center in Memphis, and obtained a Masters in Health Administration from the University of Kentucky Martin School of Public Policy and Administration. She is also a Board Certified Pharmacotherapy Specialist.

Jeannie Lee, PharmD, BCPS, BCGP, FASHP

Jeannie LeeDr. Jeannie Lee is Assistant Dean of Student Services and Associate Professor in the Department of Pharmacy Practice & Science at the University of Arizona (UA) College of Pharmacy. She is also Clinical Associate Professor in the Division of Geriatrics, General Internal Medicine & Palliative Medicine at the UA College of Medicine, and research associate and faculty in the Arizona Center on Aging. Dr. Lee has board certifications in pharmacotherapy and geriatric pharmacy, and is a Fellow of the American Society of Health-System Pharmacists (ASHP). She practices interprofessionally at the Banner University Medical Center Geriatrics Clinic and serves as faculty liaison to the Southern Arizona VA Health Care System. She represented pharmacy on the Interprofessional Leadership Team in the HRSA-funded National Center for Integrative Primary Healthcare. Dr. Lee is currently working on the NIH-funded “Improving Hypertension Medication Adherence for Older Adults” project in collaboration with UA College of Nursing and University of Illinois, Urbana-Champaign. Dr. Lee served as PI on “Geriatric Patient Care by U.S. Pharmacists in Healthcare Teams: Systematic Review and Meta-analyses” published in the Journal of American Geriatrics Society. She served as co-PI in the “Demonstrating Pharmacists’ Impact as Team Member on Therapeutic, Safety, Humanistic, and Economic Health Outcomes: A Systematic Review and Meta-Analyses” project, with primary results published in Medical Care. She also authored Pharmacy competencies for interprofessional integrative healthcare education published in the American Journal of Pharmaceutical Education. During her position at the Walter Reed Army Medical Center in Washington, DC, Dr. Lee conducted the Federal Study of Adherence to Medications in the Elderly (FAME) trial and published the findings in JAMA. A graduate of the University of Georgia, College of Pharmacy, Dr. Lee completed her residency training at the VA Medical Center in Washington, DC. Her multiple honors include the U.S. Army Commander’s Award for Civilian Service (twice received), Ralph D. Arnold Army Pharmacy Research Award, Mel Liter Clinical Pharmacy Award, Association of Military Surgeons of United States’ Circle of Excellence Award, ASHP Foundation’s Pharmacy Practice Research Literature Award (three times received), American Association of Colleges of Pharmacy Innovative Adherence Educators Challenge Award, Commission for Certification in Geriatric Pharmacy’s Excellence in Geriatric Pharmacy Practice Award, and UA College of Pharmacy Clinical Science Educator of the Year (twice received). Dr. Lee is a selected member of the National Institute on Aging Health Disparities Resource Persons Network and Chair of the Pharmacist Section in the American Geriatrics Society. She is passionate about research on medication adherence in aging and underserved populations; and interprofessional education, practice and research; and academic leadership.

Michelle MoseleyDr. Michelle Moseley, Pharm.D., BCGP, BCPS, is a clinical pharmacy specialist in geriatrics at the Veterans Affairs Tennessee Valley Healthcare System (VA TVHS) where she works within the geriatric patient-aligned care team. Additionally, she is the clinical lead for ambulatory care clinical pharmacy services at VA TVHS. Her background includes academia and geriatric acute medicine. Heather Ourth

Heather Ourth, Pharm.D., BCPS, BCGP, FASHP is the Pharmacy Benefits Management Assistant Chief Consultant for Clinical Pharmacy Practice Program and Outcomes Assessment with the Department of Veterans Affairs. In this role, Dr. Ourth’s work has an emphasis on program development, implementation and outcomes/impact assessments designed to evaluate the impact of clinical pharmacy services on the costs and quality of patient care. She also works to identify proven, innovative roles for pharmacists, lead clinical pharmacy health services research and quality improvement projects, author numerous publications, and apply pharmacoeconomic principles to support VA initiatives in this area.

Heather received her B.S. in Pharmacy from Ferris State University in 1995 and her Doctor of Pharmacy Degree from the University of Florida in 2007. She is a Board Certified Pharmacotherapy Specialist, Board Certified Geriatric Pharmacist, and a Fellow of ASHP. Dr. Ourth was a past recipient of the ASHP Board of Directors’ Award of Excellence, VA Under Secretary for Health Pharmacy Benefits Management Innovation Award, the National Alliance of State Pharmacy Associations’ (NASPA) Excellence in Innovation Award and the Michigan Distinguished Young Pharmacist Award. Dr. Ourth is a long-standing member of several professional associations including ASHP, ACCP, APhA and the Iowa Pharmacy Association (IPA) where she previously served on the Board of Trustees. 

Ashley Schenk, PharmD, BCPS, BCCP

Ashley SchenkDr. Ashley Schenk, originally from Prague, Nebraska, received her Doctor of Pharmacy degree from the University of Nebraska Medical Center in May 2015. She completed her PGY1 Pharmacy Residency at Saint Joseph East in Lexington, KY, and went on to complete a PGY2 Cardiology Pharmacy Residency at UK HealthCare in Lexington, KY. Since completion of residency, she has practiced as a Cardiology Clinical Pharmacist at UK HealthCare, where she provides pharmaceutical care to medical cardiology patients. Dr. Schenk is actively engaged in both teaching and precepting activities, including precepting pharmacy students and residents on clinical rotations and leading didactic therapeutic lectures at the University of Kentucky: College of Pharmacy. She is board certified in pharmacotherapy and cardiology. Her research involves the impact of pharmacist-run transitions of care programs on hospital readmission rates in the setting of acute coronary syndrome. Her practice areas of interest include acute coronary syndromes, heart failure, anticoagulation, and congenital heart defects.

Dr. Maria Shin obtained her Doctorate of Pharmacy from Ernest Mario School of Pharmacy at Rutgers University in 2008. Following graduation, Dr. Shin completed a PGY-1 Pharmacy Practice Residency at Kingsbrook Jewish Medical Center in Brooklyn, NY. She then completed a PGY-2 Geriatrics Residency at Central Arkansas Veterans Healthcare System in Little Rock, Arkansas. Following her residency trainings, Dr. Shin was an assistant professor for the Clinical and Administrative Sciences Department at Sullivan University College of Pharmacy in Louisville, Kentucky from 2010 until 2019. She currently is a Clinical Pharmacy Specialist in Internal Medicine at the Robley Rex VA Medical Center in Louisville, Kentucky.
